\nThe PRX T33 peel is the latest generation of TCA peel. It effectively stimulates skin regeneration without causing epidermal exfoliation or other side effects. Unlike other TCA peels, its active ingredient is combined with hydrogen peroxide (H2O2).
\nThe presence of hydrogen peroxide has a dual effect:

While complications are rare, the risk of capsular contracture is always present. To limit the risk of \"hardening\" or implant rotation, Dr Kron uses polyurethane breast implants.
\nWhat is capsular contracture?
\nWhen a breast implant is placed during breast surgery, the body reacts by forming a shell around the implant. This is a natural response of the body to a \"foreign\" implant. This shell, formed by the body's fibrous tissue, is called a \"capsule,\" \"capsular tissue\" or \"fibrous capsule.\"
\nIn most cases, this capsule remains thin and has no effect on the aesthetic result of the procedure: the breasts remain \"naturally\" soft and the bust harmonious. More rarely, the capsule contracts, thickens and hardens. The breasts then become too firm and may appear misshapen. This phenomenon is known as capsular contracture, or \"hardening.\" It can sometimes be addressed with anti-inflammatory treatments or through revision surgery. In rarer cases, implant removal may be necessary.
\nReduced risks thanks to polyurethane implants
\nStudies conducted over more than 20 years have shown that the risk of complications related to capsule formation is drastically reduced by the use of polyurethane breast implants.
\nThe polyurethane foam surrounding the implant allows for an active healing process. Instead of a single capsule forming, which carries a significant potential for contraction, the polyurethane foam promotes the formation of numerous micro-encapsulations within the foam, which neutralizes the risk of contraction.
\nBy allowing better tissue adhesion, these implants significantly reduce the risk of implant displacement or rotation following the procedure. Tissue adhesion is substantial, and in Dr Cédric Kron's experience, this has allowed him to eliminate the risk of implant rotation, a concern typically associated with anatomical implants.
\nDr Kron also serves as an Expert at training sessions for university hospital surgeons and speaks at conferences on the subject of breast augmentation with anatomical polyurethane implants.

Deeply concerned about the health of their patients, French plastic surgeons have been focused, since January 2011, on the issue of breast implant-associated anaplastic large cell lymphoma (BIA-ALCL), following information released by the American Food and Drug Administration (FDA).
\nFrench plastic surgeons work closely with their regulatory bodies (DGS, ANSM, InCA) and with international scientific societies (ASAPS).
\nWhile the topic is concerning, a sense of proportion must be kept, and the public should not be needlessly alarmed. BIA-ALCL is so rare that the risk is still very difficult to quantify. For the same reason, it is not possible to precisely identify the risk factors associated with this condition. In particular, the potential roles of implant surface texturing and genetic predisposition, both of which have been implicated, still require further study.
\nIt should be recalled that pre-filled silicone gel breast implants have been used worldwide since 1962. Accused of increasing the frequency of autoimmune diseases, they were banned in France from January 28, 1992 to January 16, 2001. However, no worldwide epidemiological study had ever managed to substantiate these accusations. It would be unfortunate to repeat such a mistake today under the influence of emotion.
\nIn both reconstructive and cosmetic surgery, the benefits that breast implants provide to patients currently far outweigh the risk of BIA-ALCL. This risk is so low that it does not justify preventive removal, regardless of the type or age of the implant. However exceptional, this risk is now part of the systematic information given to candidates for breast implantation, and all French plastic surgeons are familiar with the screening procedure.
\nThey take this opportunity to remind everyone that all women with breast implants must be systematically and regularly monitored. In the absence of clinical or radiological abnormalities, however, there is no need to change the frequency or methods of annual monitoring.

\nDear Madam,
\nThe AFSSAPS has decided to exclude cosmetic breast augmentation as an indication for injectable filler products such as hyaluronic acid (Macrolane). This decision was prompted by the results of a recent study, which highlighted risks of disruption to imaging scans and difficulties in palpating the breasts during clinical examinations. According to the study, these risks are increased with repeated injections.
\nThe consequences of these risks include a possible disruption to breast cancer screening.
\nThis decision applies the precautionary principle very strictly. As the AFSSAPS itself acknowledges, no case in the clinical studies, the literature, or Macrolane's post-approval surveillance database has revealed any new adverse effect or risk since Macrolane was launched on the market in 2008. Thanks to its effectiveness and good tolerability, Macrolane has brought great satisfaction to more than 15,000 patients worldwide.
\nRegarding the specific case of patients who have been treated by me:
\nYou should continue your clinical monitoring at the frequency I recommended based on the timing of your injection with me. Please note that should a mammogram and/or breast ultrasound be needed, I recommend having it performed by Dr LEBAR, a radiologist who co-authored this letter with me and who runs a leading radiology practice in breast cancer screening. We have worked together from the very beginning on the clinical examination, screening and analysis of mammograms and ultrasounds for patients treated with Macrolane, so Dr LEBAR has extensive experience reading these images. This poses no difficulty for the diagnostic screening of breast cancer when combining mammography and breast ultrasound.
\nPlease also note that Macrolane is fully reabsorbed within 18 to 24 months, after which you will be able to undergo mammograms and ultrasounds in the same way as other women.
\nIn conclusion, if you have any concerns, I recommend a clinical examination with me and a mammogram and/or ultrasound with Dr LEBAR. Please note, however, that the AFSSAPS confirms that women who have had breast augmentation with Macrolane-type injectable products do not require any particular medical follow-up beyond what I had already planned for you. They should continue to attend the monitoring appointments scheduled in this way. If an ultrasound or MRI is needed, it is important to inform the radiologist of the date of your injection.

\nN.B.: Today, for breast augmentation treatment, the only option available to you is augmentation with breast implants (breast prostheses).
\nIndeed, fat grafting is also not recommended because, while it does not currently interfere with screening, fat cells release growth factors, which could potentially accelerate the development of breast cancer.

\nThe breast implants concerned are pre-filled silicone gel prostheses manufactured by the company Poly Implant Prothèse (PIP), for which the Afssaps issued a decision to suspend the marketing and use of pre-filled silicone gel breast implants manufactured by PIP, along with a product recall. Indeed, this company used a different gel from the one that had been declared when the implants were placed on the market.
\n2. What is the problem with these implants?
\nFollowing vigilance reports showing an increase in the number of implant ruptures reported to the Afssaps, an inspection was carried out by the Afssaps at PIP's facilities. The findings from this inspection showed that implants had been filled with a silicone gel different from the one declared by the company when it placed the product on the market. These implants therefore do not comply with current regulations and have not undergone the required evaluation. Testing of this gel is currently being carried out by the Afssaps. The results of this testing will be communicated as soon as they are available.

Any additional fees are the responsibility of the patient or their supplemental insurance, in full or in part.
\nA cosmetic quote or notice of additional fees is provided to the patient following the pre-operative consultation, allowing them to inquire about possible coverage by French social security and their supplemental insurance.
